Before arriving for your first appointment, please take the time to download, print, and fill out the following forms. You may need to install the Adobe Acrobat Reader in order to view and print the forms.

Please bring the forms with you to your appointment, filled out completely and signed. Or if you prefer, plan on arriving about 15 minutes early for your first appointment so that you can fill out the forms in the office. If you have any questions please contact our phone representative at 1-888-718-7226 where you can leave a message for us to call you back.

 

If you are under a physician’s care, be sure and let us know.  In some cases we may need to avoid massage on certain areas, or avoid massage altogether.  This doesn’t happen very often, but it is important that we are aware of any medical conditions you may have.
